DESCRIPTION:A self-care management programme run in conjunction with the HSE\, over a 6 week period for 2.5 hours @ each session. \nThis programme is for people with long-term chronic conditions (cancer\, asthma\, diabetes\, arthritis etc.) who would welcome an insight into how diet and exercise can improve the condition and tools to help with decision making\, dealing with  health care advisers and problem solving. \nThe sessions are facilitated by a person living with a chronic condition who has been trained to provide the programme. \n

DESCRIPTION:A self-care management programme run in conjunction with the HSE\, over a 6 week period for 2.5 hours @ each session. \nThis programme is for people with long-term chronic conditions (cancer\, asthma\, diabetes\, arthritis etc.) who would welcome an insight into how diet and exercise can improve the condition and tools to help with decision making\, dealing with  health care advisers and problem solving. \nThe sessions are facilitated by a person living with a chronic condition who has been trained to provide the programme.

Time is allocated during the programme for problem solving\, feedback on diabetes self management behaviours and goal settling. Diabetes knowledge and skills are reinforced and the responsibility for the personal self management of diabetes is gradually strengthened. \nTopics covered in CODE include diabetes explained\, risk factors\, signs and symptoms\, healthy eating\, physical activity\, personal goal setting\, complications and the necessity of regular follow up and the importance of optimal diabetes control. The six month session looks at the review of achievements and personal goal setting. Our flexible approach allows the sessions to be tailored to meet the self identified needs of each group. \n
