Time is allocated during the programme for problem solving\, feedback on diabetes self management behaviours and goal setting.  \nThe programme is facilitated by Jenny Dunbar\, Diabetes Nurse Specialist and consists of 4 sessions of 2 hour duration  The first 3 sessions take place over 3 consecutive weeks \nWeek 1: Monday 28th November\nWeek 2: Monday 5th December\nWeek 3: Monday 12th November  \nReview Session: Monday 12th June 2017 \nThis review session takes place six months after the start date. CODE is free of charge to both the patient and the health care professional. \nTopics covered in CODE include diabetes explained\, risk factors\, signs and symptoms\, healthy eating\, physical activity\, personal goal setting\, complications and the necessity of regular follow up and the importance of optimal diabetes control. The six month session looks at the review of achievements and personal goal setting.
