SUMMARY:GALWAY CITY - Type 2 Diabetes Education (CODE)
DESCRIPTION:CODE is Diabetes Ireland’s  group education programme for people with Type 2 Diabetes or diagnosed Pre-diabetes.  CODE aims to deliver general information about diabetes self-management in a local community group setting. CODE allows people with diabetes to talk in a supportive atmosphere about their own diabetes. Time is allocated during the programme for problem solving\, feedback on diabetes self management behaviours and goal setting. \nThe programme is facilitated by Ms Elaine Newell\, Diabetes Nurse Specialist and consists of 4 sessions of 2 hour duration  The first 3 sessions take place over 3 consecutive weeks \nWeek 1: Monday July 3rd from 9 am to 11.30am \nWeek 2: Monday July 10th  from 9 am to 11.00 am \nWeek 3: Monday July 17th  from 9 am to 11.00 am \nReview Session: Monday December 18th from 9 am to 11 am \nThis review session takes place six months after the start date. \nCODE is free of charge \nTopics covered in CODE include diabetes explained\, risk factors\, signs and symptoms\, healthy eating\, physical activity\, personal goal setting\, complications and the necessity of regular follow up and the importance of optimal diabetes control. The six month session looks at the review of achievements and personal goal setting.
