SUMMARY:Free Type 2 Education Course- Ashbourne
DESCRIPTION:CODE aims to deliver general information about diabetes self-management in a local community group setting. The groups are kept to small numbers (11-16) allowing people to talk in a supportive\, non-judgemental environment about their own diabetes. Time is allocated during the programme for problem solving\, people’s feedback on diabetes self-management\, behaviours and goal settling. Diabetes knowledge and skills are reinforced and the responsibility for the personal self-management of diabetes is gradually strengthened. Diabetes is very much a personal condition with no one ideal plan that suits everyone. \n  \nThe programme is facilitated by a diabetes nurse or dietitian and consists of 4 sessions of 2 hours duration each and a phone call at week 10. The first 3 sessions take place over 3 consecutive weeks and the final session takes place six months after the start date. CODE is funded by Diabetes Ireland and the HSE\, therefore free of charge to attendees. \nThe sixth month session is a review of achievements and personal goals that were set and new goals are considered. SUMMARY:THURLES - Type 2  Diabetes & Pre Diabetes Education Course
DESCRIPTION:Structured education course for Prediabetes & Type 2\nCODE is Diabetes Ireland’s  group education programme for people with Type 2 Diabetes or diagnosed Pre-diabetes.  CODE aims to deliver general information about diabetes self-management in a local community group setting. CODE allows people with diabetes to talk in a supportive atmosphere about their own diabetes. Time is allocated during the programme for problem solving\, feedback on diabetes self management behaviours and goal setting. \nThe programme is facilitated by Ms.Pauline Dunne\, Senior Dietitian for Diabetes Ireland and consists of 4 sessions of 2 hour duration.  The first 3 sessions take place over 3 consecutive weeks in St Mary’s Health Centre\, Parnell Street\, Thurles\, Co Tipperary \n  \nWeek 1:           Wednesday May 30th from 10.30 to 1 pm \nWeek 2:           Wednesday June 6th from 10.30 am from 12.30 pm \nWeek 3:          Wednesday June 13th from 10.30 am from 12.30 pm \nWeek 4 :     Wednesday December 5th from 10.30 to 12.30 pm \nCODE is free of charge. \nTopics covered in CODE include diabetes explained\, risk factors\, signs and symptoms\, healthy eating\, physical activity\, personal goal setting\, complications and the necessity of regular follow up and the importance of optimal diabetes control. The six month session looks at the review of achievements and personal goal setting.
